If you can help, we are now offering you the chance to sponsor one or more of the new tiles in memory of a loved one or special event in return for a minimum donation of £10 per tile.

If you would like your name or a dedication to be written on a tile, or to be recorded in a Book of Thanks that will be kept permanently in the Church, please set out your chosen wording on the form below. We will confirm your requests when we receive your payment.

If you prefer not to use this form, you can collect a printed form from the church.
